Application Procedures<\/strong><\/p>\n<p style=\"text-align: left;\">Eligible faculty and staff who wish to enroll in the annuity insurance program should follow the procedures below:<\/p>\n<p style=\"text-align: left;\">(1) Visit the Association\u2019s official website and navigate to <strong>Self-Directed Investment \u2192 In-Service Annuity Insurance \u2192 Annuity Insurance Overview \u2192 Application Forms<\/strong>. Complete and submit the <strong>Personal Data Collection Consent Form<\/strong>.<\/p>\n<p style=\"text-align: left;\">(2) After the Association verifies eligibility (currently employed and aged 55 or above), a list of eligible applicants will be provided to the insurance company. A designated insurance representative will then contact each applicant individually to explain the policy benefits and assist with completing the application documents and signing the required consent forms (two copies).<\/p>\n<p style=\"text-align: left;\">(3) Once the annuity insurance account has been established, the insurance company will notify the applicant. The applicant must then log in to the <strong>Self-Directed Investment Platform<\/strong> and select the number of existing investment units to be transferred to the annuity insurance product. Upon receiving instructions from the Association, the trustee bank will remit the corresponding funds to the insurance company to complete the enrollment process.<\/p>\n<p style=\"text-align: left;\"><strong>3. Important Reminders<\/strong><\/p>\n<p style=\"text-align: left;\">Applicants must complete all of the following steps to finalize their enrollment:<\/p>\n<ul style=\"text-align: left;\" data-spread=\"false\">\n<li>Accurately complete all required information in the Personal Data Collection Consent Form.<\/li>\n<li>Complete and sign all required insurance application documents.<\/li>\n<li>Complete the transfer of existing investment holdings through the Self-Directed Investment Platform.<\/li>\n<\/ul>\n<p style=\"text-align: left;\"><strong>4.
